Two items describe symptoms that the patient identifies as important. One item addresses activities of daily living that are affected by illness, and in the final item, the patient rates his/her general well-being.

Scaling of items

Each item asks the patient to respond on a 7-point scale (0=as good as it could be, 6=as bad as it could be) with recall reflecting the past week.

The profile/total score is calculated as the mean of the completed items.

Validity

Construct validity was demonstrated through greater differences in mean change in symptoms for acute vs. chronic conditions. Criterion validity was demonstrated through correlations with SF-36 scores.1

Responsiveness

For each symptom and the profile score, there was a linear relationship between change in score and rating of perceived change in clinical condition over 2 weeks (significance not reported). 1
