Two items describe symptoms that the patient identifies as important. One item addresses activities of daily living that are affected by illness, and in the final item, the patient rates his/her general well-being.

Scaling of items

Each item asks the patient to respond on a 7-point scale (0=as good as it could be, 6=as bad as it could be) with recall reflecting the past week.

Validity

Construct validity was demonstrated through greater differences in mean change in symptoms for acute vs. chronic conditions. Criterion validity was demonstrated through correlations with SF-36 scores.¹
