  Self-administered questionnaire designed to measure level of distress caused by Asthma.
• Asthma Quality of Life Questionnaire [Juniper's]
  A disease-specific health-related quality of life instrument that covers both physical and emotional inpact of disease. The instrument should not be confused with the measure developed by Marks and colleagues, which carries the same name.
• Asthma Quality of Life Questionnaire [Marks']
  Self administered questionnaire intended for use with adults that covers both physical and emotional impact. Should not be confused with measure of the same name developed by Juniper and colleagues.
• Asthma Technology of Patient Experience (Asthma TyPE)
  Asthma-specific quality of life instrument
• Calgary Sleep Apnea Quality of Life Index
  A condition-specific HRQL measure designed to evaluate aspects of quality of life important to patients and that could be improved with treatment.
• Cancer Rehabilitation Evaluation System
  Cancer-specific quality of life instrument. Patients rate cancer-related problems encountered on a daily basis.
• Cantril's Self-Anchoring Ladder
  A measurement technique that asks people to rate their present, past, and anticipated future satisfaction with life on a scale anchored by their own identified values.
• Child Health Questionnaire - Parent Form 50
  Generic HRQL questionniare for pediatric patients. Parents of the patients complete the questionnaire.
• Childhood Asthma Questionnaires
  Three separate instruments. This pediatric asthma scale has separate versions for children aged 4-7, 8-11, and 12-16. Covers quality of living and level of distress caused by the disease.
• Children's Health Survey for Asthma
  A condition-specific, self-report functional health measure for parents of children 5 to 12 years of age with chronic asthma.
• Chronic Obstructive Pulmonary Disease Activity Rating Scale (CARS)
  Disease-specific instrument designed to measure life-related activity. Developed for use by patients with chronic obstructive pulmonary disease.
• Chronic Respiratory Disease Questionnaire-CRQ
  The CRQ is an interviewer administered questionnaire measuring both physical and emotional aspects of chronic respiratory disease.
• Cystic Fibrosis Questionnaire (CFQ); Cystic Fibrosis Questionnaire Revised (CFQ-R)
  Disease-specific instrument designed to measure impact on overall health, daily life, perceived well-being and symptoms. Developed specifically for use in patients with a diagnosis of cystic fibrosis. Three versions of the instrument have been developed: one for adults and adolescents 14 years of age and older (CFQ Teen/Adult); two for assessing children ages 6-13 years, one to be completed by the child and one to be completed by parent (CFQ Child and CFQ-Parent respectively).
• Daily Diary Card (Geddes)
  Chemotherapy-focused instrument designed for use in small cell lung cancer. Patients answer questions about their symptoms and general state daily.
• Daily Diary Card (Medical Research Council)
  Cancer-specific quality of life instrument. Card to be completed daily to examine how general health varies during and after treatment. Development exclusively in patients with lung cancer.
• Dartmouth COOP Charts
  Generic functional status measure designed for clinical use in primary care. Used subsequently in many different patient populations and settings.
• European Organization for the Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30)
  Cancer-specific core questionnaire for use in relation to various cancers. May be used in conjunction with the lung cancer-specific EORTC QLQ-LC13.
• European Organization for the Research and Treatment of Cancer Quality of Life Questionnaire and Lung Cancer Module (QLQ-LC13)
  Supplementary lung cancer-specific questionnaire to be used in conjunction with the EORTC QLQ-C30.
