• Pediatric Allergic Disease Quality of Life Questionnaire (PADQLQ)
  Measure of health-related quality of life that encompasses effects of allergic disease on a child's or teenager's eyes, ears, nose, lungs, skin, emotions, and everyday activities.
• Pediatric Asthma Caregiver's Quality of Life Questionnaire
  The PACQLQ is designed to measure the limitations and anxieties faced by caregivers of children with asthma.
• Pediatric Asthma Quality of Life Questionnaire
  A disease-specific health-related quality of life instrument developed by E. Juniper and colleagues that taps both physical and emotional impact of disease, designed for children ages 7-17.
• Pediatric Rhinoconjunctivitis Quality of Life Questionnaire
  A measure of health-related quality of life designed from 6 to 12 year olds with rhinoconjunctivitis.
• Perceived Quality of Life Scale
  An overall quality of life instrument developed and tested in patients receiving care in the ICU. Comes in both interview and self-administered formats.
• Pulmonary Functional Status & Dyspnea Questionnaire
  The PFSDQ is a functional status and symptom questionnaire for use in adult pulmonary patients.
• Pulmonary Functional Status & Dyspnea Questionnaire-Modified Version PFSDQ-M
  The PFSDQ-M is a shortened version of the PFSDQ with the addition of a fatigue domain. The PFSDQ-M is self administered measuring functional status, dyspnea and fatigue in pulmonary patients.
• Pulmonary Functional Status Scale
  The PFSS is a functional status instrument for use in adult patients with pulmonary patients.

• Quality of Life in Respiratory Illness Questionnaire

• Quality of Life Questionnaire for Cancer Patients Treated with Anti-Cancer Drugs (QOL-ACD)
  Developed to assess quality of life of Japanese cancer patients undergoing chemotherapy.
• Quality of Well Being
  An interviewer-administered general health-related quality of life questionnaire measuring symptoms, mobility, physical activity and social activity. Scores can be translated into economic evaluation for cost-effectiveness studies or quality of adjusted life years.
• Quebec Sleep Questionnaire (QSQ)
  A disease-specific health-related quality of life instrument designed for use with adults to assess the effect of obstructive sleep apnea on quality of life.
• Questions on Life Satisfaction - Cystic Fibrosis (FLZM -CF)
  Generic measure of functional status in children including children with chronic physical conditions.

  Cancer-specific tool to measure psychological and physical distress in cancer patients participating in clinical research. Validation in various cancer populations.
• Seattle Obstructive Lung Disease Questionnaire
  The SOLQ is a brief, computer-scannable, self-administered questionnaire designed to measure the physical function, emotional function, coping skills, and treatment satisfaction of patients with COPD.
• Self-Administered Dependency Questionnaire
  A clinical measure of child dependency on parents, particularly mothers. Measures child independence in non-illness activities.
• Severe Respiratory Insufficiency Questionnaire (SRI)
  Health related quality of life measure. Developed for use by patients with respiratory failure from a variety of underlying diseases receiving mechanical ventilation.
• SF-12 Health Survey
  Generic Quality of Life measure derived from the SF-36
• Sickness Impact Profile
  Generic measure used to evaluate the impact of disease on both physical and emotional functioning. Patients are asked to respond to the items as they are on that day. The measure has also been used in patients with COPD and Asthma.
• Sino-Nasal Outcomes Test
  A measure of health-related quality of life in adults with rhinosinusitis. Also available are the original longer version (RSOM-31) and a shortened version (SNOT-20).
• Smoking Cessation Quality of Life (SCQoL) Questionnaire
  The SCQoL assesses change in well-being and functioning associated with smoking cessation. It consists of generic and condition-specific components.
• St. George's Respiratory Questionnaire
  Disease-specific instrument to measure impact on overall health, daily life, and perceived well-being. Developed for use by patients with airway obstruction.
• Symptom Distress Scale
  Cancer-specific tool for assessing cancer-related symptoms. Subjects respond about how they have been feeling during the preceding week. Validation with chronic disease (primarily cancer) patients.
• Therapy Impact Questionnaire
  Instrument designed for quality of life assessment in general cancer patients with advanced disease.
